El empresario est\u00e1 obligado a informar de su lesi\u00f3n o enfermedad <strong>si no est\u00e1<\/strong> que pueda trabajar durante 5 o m\u00e1s d\u00edas. Si el empresario no comunica su reclamaci\u00f3n en un plazo de 7 d\u00edas naturales (sin incluir domingos ni d\u00edas festivos legales) despu\u00e9s del 5\u00ba d\u00eda de incapacidad total o parcial del trabajador lesionado a la DIA, entonces puede presentar una reclamaci\u00f3n del trabajador (formulario 110). <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">El formulario 110 lo utilizan los empleados que reclaman prestaciones como consecuencia de una lesi\u00f3n o muerte relacionada con el trabajo. Debe rellenar este formulario cuando crea que no est\u00e1 recibiendo todas las prestaciones de indemnizaci\u00f3n por accidente de trabajo a las que tiene derecho.  Los formularios que se presenten sin la documentaci\u00f3n justificativa requerida ser\u00e1n rechazados. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Una vez cumplimentado, deber\u00e1 hacer dos copias. El original debe enviarse al Departamento de Accidentes Laborales. Una copia debe enviarse a la compa\u00f1\u00eda de seguros y otra copia debe conservarse para su propio archivo.
