Si decide apelar, deber\u00e1 comunicar la siguiente informaci\u00f3n:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Fecha de la lesi\u00f3n<\/li><li>El primer d\u00eda natural de trabajo perdido<\/li><li>El 5\u00ba d\u00eda natural de trabajo perdido<\/li><li>La compa\u00f1\u00eda de seguros de accidentes de trabajo<\/li><li>Las partes del cuerpo afectadas y los tipos de lesiones<\/li><li>Qu\u00e9 tipo de prestaciones busca<\/li><li>Cu\u00e1nto tiempo podr\u00eda estar sin trabajo (si procede)<\/li><li>D\u00f3nde acudi\u00f3 inicialmente a recibir tratamiento<\/li><li>El nombre del m\u00e9dico que le atiende actualmente<\/li><\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Si le deniegan la indemnizaci\u00f3n por accidente de trabajo, necesitar\u00e1 3 copias de <a href=\"https:\/\/kecheslaw.com\/es\/areas-de-practica\/indemnizacion-de-los-trabajadores\/faqs-2\/que-es-forma-110\/\">Formulario 110<\/a>. Una copia para la DIA, otra para la compa\u00f1\u00eda de seguros y otra para sus propios archivos (o para un <a href=\"https:\/\/kecheslaw.com\/es\/injury-attorneys\/\">abogado<\/a> que posteriormente decida contratar). Env\u00ede el original de su formulario 110 (Reclamaci\u00f3n del Empleado) firmado, as\u00ed como uno de los siguientes documentos que apoyen su caso a la DIA. Dichos documentos son:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Facturas m\u00e9dicas impagadas<\/li><li>Informes m\u00e9dicos<\/li><li>Cualquier informe que apoye c\u00f3mo se produjo el accidente<\/li><li>Los nombres de los testigos<\/li><li>Las declaraciones de los testigos<\/li><\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">Despu\u00e9s de apelar a su reclamaci\u00f3n y enviarla a la DIA, ser\u00e1 revisada y har\u00e1n una de estas dos cosas. La DIA le enviar\u00e1 una carta de rechazo (en la que se indica qu\u00e9 informaci\u00f3n falta o es incorrecta) o, si su informaci\u00f3n es correcta, usted, su empleador y la compa\u00f1\u00eda de seguros recibir\u00e1n una notificaci\u00f3n. Esta notificaci\u00f3n es para concertar una reuni\u00f3n de conciliaci\u00f3n para hablar de su reclamaci\u00f3n.
